Please fill out the following online application for NEW dogs or cats only. Dogs must be a minimum of six months old to be issued a new license. Please submit this online application to the Municipal Clerk’s Office along with the following:

• Your online payment submission to the Township of Fairfield.
• Copy of Current Rabies Vaccination Certificate (must be valid no less than 10 months of the current licensing year).
• Copy of Spay/Neuter Certificate (if you wish to qualify for a $3.00 reduction in license fee). Proof of Spay/Neutered must only be supplied one time; it will be on your pet’s permanent record thereafter.

Vaccination and license requirements: No person shall own, keep, harbor or maintain any cat or dog over seven months of age within the municipality unless such cat or dog is vaccinated and licensed.

* Written proof of current rabies vaccination which must be good for 10 months of licensing duration making the rabies cutoff date 2/1 of the licensing year. Written proof of spaying or neutering will be needed for price reduction. Please have and provide this information when applying.

As a courtesy a renewal notice is mailed January 1^st for dogs and April 1^st for cats provided the pet was licensed in the prior year.
